Premier Physician Group works to help primary care physicians spend more time with their patients, providing more focused, effective care. Our hospitalists and preferred providers also collaborate with your primary doctor when you need care outside his or her office. Working together we help ease your transition from hospital to home, ensuring you are well cared for every step of the way.

Our Circle of Care

When you are admitted to a partner hospital they are assigned to one of our hospitalists for care. The hospitalist acts as your dedicated physician while you are in the hospital, overseeing your care and ensuring all treatment and notes are entered into our data system so that your primary care physician is always in the know. Once your treatment in the hospital has concluded, we will help you either transition home or to a post-acute care facility as appropriate.

When you are discharged from the hospital but aren’t quite ready to go home, you will be transitioned to one of our post-acute care partners, like a skilled nursing facility, long-term acute care facility, hospice facility or rehabilitation facility. We will inform the partner facility of any care and treatment received in the hospital and work with them to coordinate further care. Again, information on your care, treatment and any other medical notes are entered into our data system so that your primary care physician is always in the know. The goal of a hospice facility is to provide patients with compassionate, dignified end-of-life care. Skilled nursing, long-term acute care and rehabilitation facilities work to help patients rebuild the health and strength they need in order to be safely discharged to their homes.

When you have had successful treatment in hospital and/or a post-acute care facility and are ready to go home we will notify your PCP. A summary of all your care within our facilities will be provided to your PCP as well so that he or she will be fully informed of your treatment and condition. Your PCP will then provide you with any necessary follow-up care needed to help you finish your recovery so that you can regain your health and enjoy life at home.
